Please utilize this page to upload your enrollment documents. This may include enrollment forms, a copy of your valid photo ID, or any other documentation your CANARX representative has asked you to submit. Your documents will be securely uploaded over a full encrypted and secure connection.

Please Note: We accept original prescriptions by mail from the prescriber or from the member. We only accept faxes directly from the prescriber’s office.

Valid forms of government issued photo IDs include: 

• Driver’s License (Issued within U.S. including U.S. Territories)
• Non-Driver Photo ID (Issued within U.S. including U.S. Territories)
• U.S. Passport
• U.S. Military ID Card
